The Aurum Institute is offering a great opportunity to suitably qualified and experienced staff to contribute towards our mission to improve the health of people and communities through innovation in Health Research and Systems. We are focused on research, support and implementing integrated approaches to managing TB and HIV. The following positions have become available as part of the various projects currently being run by us and you are welcome to apply should you meet the requirements of the position. NB: Successful candidate will be supporting the following areas:1. Saldanha.2. Vredeburg.3. Langebaan.4. Hopefield.5. Laingville.The Linkage officer will be responsible to actively assist the DoH TB program in identifying TB symptomatic patients and assist with Lost to follow up TB confirmed patients within communities and re-enter them into care, treatment, and support. Additionally, they will be responsible for managing Aurum CHWs and support existing CBOs/CHWs with TB contact tracing, household screenings, health awareness campaigns and establishing sustainable linkage platforms within Department of Health (DOH). Technical Work ResponsibilitiesLinkage with stakeholders (Between DOH facilities and Aurum Roving team).Strengthening linkages between the community and DoH facilities in collaboration with the Roving team. Assist in identify individuals who has TB symptoms or are HIV-positive but are not in medical care by receiving referrals, utilizing information about missed appointments, and conducting outreach activities at various community venues. Serve as a point of contact and information base between the community and DoH facilities.Provide accurate information to patients about living with TB and HIV, preventing further transmission, the benefits and challenges of TB and HIV care and treatment, and available services and how to access them.Aid patients to help them enrol or re-engage in medical care.Together with case managers and clinical staff, support patients throughout the early part of their care so they participate fully, become educated, and can advocate for themselves.Together with DOH and Aurum project coordinator identify TB hotspots and participate in arrangement for community outreaches (awareness campaigns) to these identified areas.Comply with all programme protocols, including those regarding confidentiality of patient information. Support roving team with data collection, collation and verification related to linkage of clients to care. Report on activities completed to DOH and Aurum Project Coordinator.Track linkage targets, prepare and submit monthly reports.Responsible to submit Roving teams HR administration documents weekly. DutiesDaily linkage with DOH health facilities to identify patients that need to be followed up or contacts to be screened for TB/HIV.Linkage with Aurum Project Coordinator to arrange outreach activities in the community. To perform miscellaneous job-related duties as assigned including support for other programs related to entry into TB and HIV care. Collaborate with community-based organizations and DOH clinics to facilitate transition of participants into care.Input accurate and complete data for all contacts with clients into the project database.Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, evaluations, and Aurum HR related documentation.Participate in regular staff meetings, staff training programs, supervisory sessions, and accept the responsibility for aiding the development of positive team relationships as requested.To implement quality improvement processes throughout the conduct of the projectEnsure regular and clear communication with the team.All the communication is conducted in the spirit of Aurum values (teamwork, respect, excellence, and integrity).Adherence to organogram and communication lines regarding various problemsParticipate in training meetings and other activities both on site and at central venues to accomplish aims of the project.General supervision: routine duties are performed with minimal supervision; standard practices or procedures allow me to proceed alone at routine work; occasional check of work while in progress; work is reviewed upon completion.Comply with all safety policies, practices, and procedures. Report all unsafe activities to supervisor and/or Human Resources. Adhere to the relevant procedures with regards to participant confidentiality. Perform other duties as assigned or required.
